To make an appointment with Dr. Amin or our certified physician assistant, please call our office directly at (312) 266-6462. Please be specific in what you need to address at your visit, so that an appropriate time can be set aside just for you. If you need to change the reason for your appointment, please let us know beforehand, so we can allow the appropriate time with your provider.
Not all business hours are available for both providers, but we strive to do our best to work with your schedule.
Appointment times are set aside specifically for our scheduled patients. For our return patients, please be courteous and arrive at least 10-15 minutes before your scheduled appointment time to check in, and update any insurance or demographic changes. If you arrive 15 minutes after your scheduled appointment time, please be aware that based on the provider’s schedule, you may have to be rescheduled or wait to be seen.
For our new patients, we ask that you arrive at least 20-30 minutes before your scheduled appointment time to check in and complete the new patient paperwork. Please bring your state identification card, insurance card and the required referral if needed.
When you arrive to your appointment, please check in with our receptionist and present your state identification card, your insurance card, and a referral if required. We may check your insurance card, as well as demographics at every visit, just to make sure we have the most up to date information.
We are aware that emergencies arise, but please be courteous and cancel or reschedule your appointment at least 24 hours in advance. We do make reminder calls for appointments.
A parent or guardian must accompany any patients under the age of 18 to each appointment and are responsible for consent of treatment and full payment. Unaccompanied minors cannot be seen.
