We participate in several PPO and HMO plans. It is the responsibility of the patient to verify if we are in network with their plan. Our office staff will not be able to provide such information due to the rapid changes in health care and insurance plans.
Please bring your most recent insurance card to all visits. We need to verify the most up to date information, so that the insurance company can be billed correctly.
For HMO patients, it is the responsibility of the patient and/or patient’s financial responsible party to have the correct referral at each visit. Our staff will strive to inform you when a referral is required, but it is the ultimate responsibility of the patient to have the correct paperwork. We cannot bill the HMO insurance if the correct referral is not present.
